Access To Your Medical Records

Under the terms of the Data Protection Act 1998 all patients have the right to view or request copies of their own medical records (charges may apply – see below)

Patients need to fill in and sign an access request form, you will then need to hand the form to a member of the reception team who will then arrange your appointment or copies of your medical records.

Please allow 5 working days from receiving the access form to your records being ready for collection.Medical Records
